Step 1: Getting Started
The “Acrobat” reader will be needed to view and / or print the forms. If you do not have it, please click on the following icon for a free download
Step 2: How to Fill In Forms On-Screen
By using Acrobat Reader (version 4.0 or higher), applicants may proceed in two ways:
1) Download the complete forms (Form 100 – 5 pages) (Form 101 – 7 pages), and fill in, on-screen:
Form 100 – Complete Form (revised April 2004; Appendix “A” no longer required)
Form 101 – Complete Form (revised June 2010)
2) Download the forms (Form 100 and Form 101) on a page-by-page basis, and fill in, on-screen:

There are several multi-line fields in both forms. Applicants may complete data for these fields using any wordprocessor and then cut and paste to the form. Correct word wrap and conversion to the CFCAS preferred font (Times Roman 12) occurs automatically. Please note that any text which spills outside of a multi-line field will not print.
Check boxes can be checked or unchecked with one mouse click.
By using Acrobat Reader, applicants cannot save completed forms but they can print them for signature and mailing to CFCAS. By completing one page at a time, corrections and adjustments can be made before the final version is printed.
By using the full Acrobat Program (version 4.0 or higher), applicants may save completed forms on their computer.
